Online and Printable Forms
You can complete the Patient Questionnaire online through your Secure Patient Portal and entering your Username and Password. You the should click the ‘Documents’ tab and complete the Questionnaire in the ‘Forms To Be Filled out’ folder. If you need to obtain or reset Username and Password for your Secure Patient Portal please call our office at 516-487-2700 or toll free 877-7NYCRYO (877-796-2796).
If you are unable to complete the Online Forms please download and complete the Men’s Health Questionnaire. The completed questionnaire should be bought with you to your appointment.
If you are a Hospitalized Patient we will need the following to be completed:
- Please give the Quest Blood Test Form to your care team. They will arrange for the blood to be drawn and sent to Quest Laboratory which for the required FDA approved tests. Please note that Quest Laboratories participates in almost all insurances. You can check with them by calling the Quest Customer Center at (866) 697-8378 to confirm.
- Please complete the Hospitalized Patient Information Packet and have the completed packet brought with the sample to the office.
